Family Practice/Primary Care Nurse Practitioner

Salary Ranges between: $148k-$155k (varies depending on years of experience) We re seeking a compassionate and motivated Nurse Practitioner (FNP) or Physician Assistant (PA) to provide high-quality care in an outpatient community health setting. What You ll Be Doing: Conduct comprehensive patient evaluations, including physical exams and health assessments Collaborate with physicians and specialists to ensure coordinated, well-rounded care Create and manage individualized treatment plans tailored to patient needs Prescribe medications and monitor treatment effectiveness Support patients with both primary care and behavioral health concerns Coordinate referrals to specialty providers and external services Contribute to the development and improvement of clinical care processes Requirements: Active New York State license as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) New grads are welcome to apply!
