How you'll contribute A Clinical Staff Pharmacist who excels in this role: * Evaluate patient drug profiles to optimize therapy and ensure safety. * Collaborate with physicians and healthcare providers to improve medication therapy outcomes. * Participate in medication reconciliation to ensure accurate patient medication histories. * Provide pharmacokinetic assessments and therapeutic drug monitoring. * Attend interdisciplinary team rounds and meetings to support collaborative care planning. * Promote safe, appropriate, and cost-effective medication use.
