Schedule/Hours 40 hours per week Monday to Friday, 8-hour shifts (5x8) Call schedule includes 2 to 4 weekday nights per week Weekend coverage required 1 to 3 weekends during the 13-week contract Responsibilities Provide occupational therapy in a home health setting focusing on community-based care Collaborate with the multidisciplinary team to develop and implement patient care plans Manage a typical caseload of 4-5 patients during 8-hour shifts or 5-7 patients during 10-hour shifts Support inpatient care on Med/Surg floors as needed Qualifications or Desired Experience Minimum of 2-3 years of experience in home health or similar healthcare environments Hospice care experience preferred; training available if needed Valid BLS and ACLS certifications Additional Information The role requires the ability to work certain holidays, including Thanksgiving and Christmas Providing care exclusively in community settings; no services in assisted living facilities This contract assignment offers a valuable chance to work in a dynamic healthcare setting with a supportive team.
