Family Practice/Primary Care Nurse Practitioner

In this role, you will provide evidence-based care to adiverse patient population with an emphasis on prevention, chronic diseasemanagement, and whole-person health. You will practice in a collaborativeenvironment alongside a dedicated Whole Health Coach and a supportive clinicalteam, with a model designed to prioritize quality of care over volume.
