The Health Coach partners closely with the medical team and Registered Dietitians to deliver evidence-based coaching that empowers patients to build healthy habits, improve overall well-being, and achieve long-term health outcomes.Essential FunctionsAssists our members with a variety of health concerns including, but not limited to, weight management, stress management, developing routines to improve nutrition, physical activity, sleep and other healthy lifestyle changesUsing a collaborative process that includes assessment, planning, implementation and evaluation, the Health Coach provides quality coaching programs that are part of an integrated and comprehensive care modelDeliver high quality coaching programs for our membersCollaborate with members and care teams to assess, develop, implement, and evaluate action plans related to achieving and maintaining an optimal health statusEngages individual members and groups in healthy lifestyle changes using evidence-based coaching techniques that align behaviors to personal values while fostering an environment of individual responsibility and accountabilityConducts individual coaching assessments and follow-up sessions on a monthly to bi-monthly basis to patientsDelivers virtual wellness classesProvides recommendations for healthy behavior changes based on established protocolsServes as a consultant and educational resource to providers and membersPerforms other duties as assignedSkillsDemonstrated ability to communicate, solve problems, and work effectively with people while using good judgment in new and challenging situationsKnowledge and proficiency in current, evidence-based methods of health coaching including motivational interviewing and cognitive behavioral therapy with an emphasis on health promotion and risk reductionEducation and ExperienceBachelor's degree in a relevant health field (i.e Health Education, Nutrition, Nursing, Public Health, Exercise Physiology, Psychology)Health Coaching certificate from an approved ICHWC organization preferredMinimum of 3 years' experience as a Health Coach/Exercise Physiologist in an out-patient settingMust be able to effectively communicate with patients and team membersPrior work-site health experienceExperience utilizing electronic health record systemsPay range estimate: $25/hr - $35/hrThe estimated range is the budgeted amount for this position.
