Join Our Dynamic Team as a Registered Nurse (RN) / Care Manager!Job Description:- Duration: 13 weeks - Shift: 8a - 4:30p with rotating weekend shifts - Requirements: - BLS certification - Recent experience in both discharge planning and utilization review (UR) in a hospital setting - Minimum 3+ years of acute hospital case management experience (preference for travel experience) - Permanent address must be +50 miles from the facility - Qualifications: - Must possess extensive hospital case management experience (not charge nurse, staff nurse, outpatient home health, or hospice case manager) - Ability to hit the ground running as an acute hospital case manager by day 2 - Proficiency in discharge planning, including transitioning patients to home health, SNF, LTAC, and IRF - Expertise in utilization review for medical necessity using MCG criteria - Knowledge of patient choice letters and IMMs in accordance with the Conditions of Participation and proper documentationWhy Catalytic Solutions (CatSol):At CatSol, we connect dedicated therapists with rewarding assignments across the country.
