Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record; and checking of IV products. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, clinical protocols per pharmacy and pharmacist interventions). Hourly Salary Range: $91.06-$106.67 Bargaining Unit: HX-UPTE Job Qualifications Press space or enter keys to toggle section visibility We are seeking an exceptionally-skilled and compassionate pharmacist with the following qualifications: * Doctor of Pharmacy Degree * California Pharmacy license * ASHP-accredited PGY2 oncology residency or at least a minimum of 1 year of pharmacy practice experience in oncology * Recent clinical oncology experience (within 5 years) * Board Certification in Oncology (BCOP) preferred * Previous infusion pharmacy experience preferred * EPIC/Beacon experience preferred * Exceptional communication, interpersonal, organizational and follow-through skills * Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations * Required to have the ability to travel to multiple work sites within UCLA Health As a condition of employment, the final candidate who accepts an offer of employment will be required to disclose if they have been subject to any final administrative or judicial decisions within the last seven years determining that they committed any misconduct; or have filed an appeal of a finding of substantiated misconduct with a previous employer.
