DEPARTMENT: PHARMACYFACILITY: Tift Regional Medical CenterWORK TYPE: Full Time: 7 on, 7 offSHIFT: DaytimeSUMMARY: The Emergency Medicine Pharmacist collaborates with other Emergency Department healthcare members to manage acute pharmacotherapy, prepare high risk medications, and guide treatment at the bedside. The Emergency Medicine Pharmacist will provide immediate, evidence-based medication expertise in the Emergency Department to optimize patient care, improve safety, and support rapid decision-making during critical situations.RESPONSIBILITIES:* Reviews orders written by physicians and authorized prescribers and enters those into the electronic health record efficiently and accurately.* Independently works up patients to determine appropriate dosing of selected target drugs or when requested by the prescriber.* Appropriately monitors the effectiveness of prescribed medications by checking lab values and other patient parameters when dictated by protocol or by individual patient needs.* Follows up with nursing or prescribers when critical information elements are needed to process medication orders.* Participates in department mandated clinical activities such as IV/PO conversions, drug renal screens, dosing verifications for pediatrics patients etc.* Documents clinical interventions as required.* Is familiar with and is able to independently initiate selected drug protocols when requested.* Effectively manages workflow during his/her shift, requests and/or provides assistance when appropriate.* Provides drug information to prescribers, nursing staff and patients when requested.* Supervises Medication Reconciliation Pharmacy Technicians.* Assists in the supervision and training of Pharmacy and Tech Students.* Facilitates proper information transfer with regard to medication use for patients admitted from the ED.* Performs other duties as assigned.* Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc. ("TRHS") policies as they presently exist and as they change or are modified.* Understands and adheres to: TRHS' compliance standards as they appear in TRHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information.* Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position.* Offers suggestions on ways to improve operations of department and reduce costs. * Attends all mandatory education programs.* Improves self-knowledge through voluntarily attending continuing education/certification classes.* Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.* Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department.* Volunteers/participates on hospital committees, functions, and department projects.* Manages resources effectively.* Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel.* Makes good use of time so as to not create needless overtime.EDUCATION:* Doctorate Degree in PharmacyCREDENTIALS: * PHARMACISTSouthwell/Tift Regional Health System, Inc. is an Equal Opportunity Employer.
