The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.Schedule: 37.5-hour work week Monday through Friday from 11:30 am to 8:00 pm.  May float to other offices as needed.  The scheduled will be determined upon hire.Location: 47-01-Queens Blvd. Sunnyside, NY 11104 (Main Office)May travel to these locations depending on coverage:  116 East 16th Street, Third Floor, New York, NY 10003 or 120 West 23rd Street, Ground Floor, New York, NY 10011, and 200 West 57th Street, Suite 702,  New York , NY 10019 Primary Responsibilities:Greets patients and visitors in a prompt, courteous and helpful mannerVerifies insurance information, address and phone number of the patient, registers new patientsArrives patients on the computer for scheduled appointmentFollow the payment Security policy and procedures according to Optum Medical guidelinesAnswers incoming phone calls in a courteous and prompt manner; directs calls and takes messagesAbility to maintain a work queue list- complete any pending actions ie referrals, scheduling appointments Schedules patients for future appointmentsAssists therapists with paperwork needed for pre-certification of therapy visitsConfirms new patient appointments for the next dayPerforms clerical duties such as copying, faxing and filingContact patient in case of no callo showAbility to use an electronic health record (EPIC)Effectively executes time management by notifying the provider team of the patient's arrival, consistently reviewing the daily patient schedule, and notifying the team of any potential conflicts., which may include voicemails, emails, and online bookings.Helps ensure continuity of care and communication between Optum providers and other partners as pletes clinic reports and administrative duties to ensure exceptional patient care. Execute working knowledge of HIPAA privacy laws by protecting patient information.
