We are looking for passionate candidates who are committed to quality leadership. Job Summary In this role, you will utilize the nursing process to provide individualized care. Your responsibilities include: Conducting assessments and sharing vital information with providers. Developing tailored patient education and care plans. Coordinating patient care across the continuum with the healthcare team. Managing disease registries and following up with patients to meet clinical goals. Promoting health and wellness initiatives. Demonstrate competency by adhering to nursing standards as set forth by the Nurse Practice Act and the American Nurses Association. Your role will help shape the quality of care at our facilities. Qualifications Bachelor's degree in Nursing is strongly preferred, with specific requirements based on your location. Graduated from an accredited nursing program. Preferred two years of clinical experience. Current RN license in the state of practice. Must maintain required competencies and certifications, including Basic Life Support (BLS) within six months of employment. Valid driver’s license is necessary.
