Home Care Nurse Practitioner

Join our dedicated team of Nurse Practitioners and Physician Assistants as we provide exceptional patient-centered care in the comfort of patients' homes. Your role will be vital in ensuring the health and well-being of our patients while enjoying a rewarding work-life balance. This position offers a Monday to Friday schedule with NO weekends. Conduct initial patient screenings and comprehensive physical examinations, along with creating thorough assessment plans. Address urgent care needs efficiently and effectively as required. Order, interpret, and analyze diagnostic tests, determining the necessity for follow-up appointments and additional treatment options. Prescribe and manage medications and treatments, including psychopharmacological agents, where appropriate. Provide compassionate palliative and, when necessary, end-of-life care to both patients and their families. Deliver high-quality care while maintaining compliance with company metrics regarding documentation, coding, HEDIS measures, patient engagement, and managing medical expenses. Qualifications: Current Nurse Practitioner or Physician Assistant license and Board Certification. Possess an NPI number. Current BLS/ACLS certification. Valid Driver's License, auto insurance, and a willingness to drive to patient locations. Competitive salary ranging from $120,000 to $166,000 based on experience, along with excellent benefits and 4 weeks of PTO.
