Thanks! -Hybrid schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm -Home care or hospice experience preferred -Chronic disease management experience preferred -Must be willing to travel up to one hour from home and visit patients in their homes up to 4 days/week -Must be bilingual in Spanish Pay flexible This is a full time position.
