This role serves as an integral member of the multidisciplinary care team, delivering pharmacotherapeutic expertise to support safe, effective, and evidence-based cancer care.The Clinical Oncology Pharmacist will provide direct patient care, collaborate closely with providers and nursing teams, support access to specialty medications (with a strong focus on oral chemotherapy), and contribute to education, research, and practice management initiatives. The position is primarily remote, with required training and occasional on-site visits in Seattle.Key ResponsibilitiesClinical ResponsibilitiesCollaborate with providers to individualize medication therapy to promote safe, effective, and cost-conscious oncology carePerform comprehensive medication therapy management and support safe transitions of careMonitor and adjust drug regimens as referred, including ordering and interpreting laboratory testsProvide refill authorization with assessment of adherence, efficacy, safety, and drug interactionsServe as a drug information resource for providers, nursing staff, research staff, and patientsEnsure patient access to specialty and oral chemotherapy medications, including providing clinical documentation to insurers as requiredIdentify opportunities to optimize therapy for under-treated oncology patientsAssess and address barriers to medication adherence, including cultural, physical, cognitive, emotional, and socioeconomic factorsEducate patients on disease self-management and oral chemotherapy regimensReview clinic prescribing patterns and provide targeted education aligned with national and institutional oncology guidelinesManage anticoagulation, infectious disease therapy, pain/palliative care, or complementary and alternative therapies under collaborative practice agreements, as applicableEducationServe as a primary preceptor for pharmacy residents and student pharmacistsProvide education to medical residents, nursing staff, and medical studentsPresent at multidisciplinary educational conferences as requestedDevelop and deliver nursing in-servicesProvide didactic instruction to pharmacy students, as requestedResearchParticipate in collaborative research projects with interdisciplinary healthcare teamsContribute work for publication in professional journalsAssist with the preparation, review, and implementation of drug study protocolsSupport investigational drug studies in collaboration with research investigatorsPractice ManagementImplement and maintain policies and procedures related to medication safety and effectivenessStay current on oncology-specific literature and practice updatesDevelop and maintain medication monitoring and documentation systemsEstablish effective communication among pharmacists, providers, nurses, and extended care teamsParticipate in pharmacy and hospital committees as requestedSupport quality assurance initiatives, including medication error preventionConduct drug utilization reviews and cost-containment analyses, when neededDispensingPerform duties of a dispensing pharmacist in oral chemotherapy, infusion, and/or retail pharmacy settings as required
