Saratoga Hospital seeks a compassionate and dedicated Geriatrician to join and help grow an established practice, Saratoga Hospital Medical Group - Geriatric Medicine, in Saratoga Springs, NY. This physician will serve as a clinical anchor in advancing the hospital's vision for a Center for Successful Aging, ensuring that older adults in the region receive exceptional, coordinated care. The position is primarily outpatient. This position allows for longer visits to allow for meaningful engagement with patients and caregivers, ensuring that all care aligns with individual goals, values, and quality-of-life priorities. Key Responsibilities: Provide comprehensive geriatric assessments and consultative care for older adults with complex medical and social needs. Collaborate closely with the hospital's inpatient consultative geriatric service to ensure smooth transitions and continuity of care across settings. Work in a consultative role with community primary care providers, offering expertise in geriatric syndromes, polypharmacy management, functional assessment, dementia care, and advance care planning. Deliver care in the outpatient clinic setting primarily with some occasional inpatient vacation coverage. Dedicate extended visit time to advance care planning, eliciting goals of care, and guiding shared decision-making. Collaborate with other members of the Geriatric team including advanced practice providers, case management, social work and other interdisciplinary care teams to optimize patient outcomes through value-based, patient-centered team-based care. Actively participate in the Geriatric Medicine Fellowship Program, providing teaching and mentorship to fellows. Opportunity also exists to engage in teaching opportunities with Family Medicine residents, based on personal interest. Play a key role in shaping, growing, and anchoring the hospital's Center for Successful Aging initiative. Qualifications: MD or DO with board certification/eligibility in Geriatric Medicine (Internal Medicine or Family Medicine trained). Strong clinical skills in geriatric medicine, including diagnosis, management, and care planning for older adults. Commitment to value-based, patient-centered care models with a consultative focus. Excellent communication and interpersonal skills with patients, families, and healthcare professionals. Interest in program development and innovation in Geriatric Medicine.
