Position: Social Care Manager and TrainerCompensation: $50,000-$52,000 annually (based on experience and credentials)Hours: 37.50 hours per weekLocation: Riverhead, NY with travel throughout Suffolk CountyPosition Description: The Social Care Manager and Trainer is a grant funded position that will help to advance the Association for Mental Health and Wellness's Social Care priorities, including working closely with the Social Care Network and other partner organizations to improve social care connections Medicaid enrollees. The position will support the development of workflows and staff training on internal procedures to ensure sustainable and long-term impact, while interpreting and training on external polices. The Social Care Manager and Trainer will also carry a small caseload of Health Home Care Management clients and conduct health-related social needs screenings, referrals to appropriate services, and enhanced care management. At the end of the grant period, this position may transition into a full-time Care Manager position, subject to position availability. A personal automobile and a clean NYS Driver's License are required; mileage reimbursement provided. Duties/Responsibilities: Participate in all required SCN trainings and meetings The development of sustainable workflows Meet established KPI's, track metrics and report progress to Executive Staff and SCN Learn and maintain a high proficiency in the Unite-Us platform Training staff to deliver these services in accordance with SCN policies Review and approve services to be billed to the SCN Coordinate and outreach to internal and external Medicaid clients Identification and verification of eligibility by utilization of appropriate screenings for clients Verification of demographic information in the documentation platform and other program documentation systems Confirmation of a client's desire to receive social care services Consent documentation Client outreach by virtual, telephonic means or in-person care setting to perform screenings, establish resource needs, connect to those resources, and follow up to determine if need is met Utilize Unite Us to complete referrals and assist navigating to the appropriate health and social care services - either existing federal, state, or local social care infrastructures or social care services covered by the waiver Qualifications: A Bachelor's degree in a human service/health discipline and five years' experience delivering direct care is preferred. Candidates with Master's degrees are strongly encouraged to apply. 2 years of supervisory experience and training facilitation. Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance abuse challenges, trauma as well as chronic physical health conditions; Strong communication skills and collaboration skills; Understanding of the key elements of recovery and wellness Sound organizational skills including computer skills Basic knowledge of the public health/mental health system, public entitlement and social service programs; Demonstrated ability to communicate effectively verbally and in writing with people of different cultural and socioeconomic backgrounds Ability to complete required trainings and additional certifications or trainings as assigned Organizational and time management skills Ability to prioritize and demonstrate flexibility in day-to-day functions Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
