This program delivers proactive, relationship-based medicine with a focus on access, prevention, and coordinated care. You'll practice in a team environment with advanced practice providers (APPs) and collaborate across more than 200 specialties across the health system. The model emphasizes population health, lifestyle medicine, and seamless care navigation-supported by a high tech, high touch infrastructure.
