Home Health/Hospice RN

RN with documented experience in Home Health and Hospice as a case manager. Must be proficient in OASIS; prefer certification/credentials with OASIS; shifts are Monday - Friday - fulltime position plus after hours and weekend on call coverage. This position requires daily travel. Reliable transportation needed when a company car is not available for use. Seeking individual who is able to assess patients in their home; identify needs for qualifying patient to Home Health or Hospice. Strong communication skills working closely with interdisciplinary team. Able to manage overall Home Health services for each patient.
