Nurse Practitioner (NP) Long-Term Care Location: PennsylvaniaSchedule: Full-Time Monday Friday, 8:00 AM 5:00 PM (40 hours/week) Job Summary We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary and preventive care to residents in skilled nursing facilities. This role focuses on comprehensive medical management, advanced care planning, and collaboration with interdisciplinary care teams to improve outcomes for patients with complex and chronic conditions. Responsibilities Provide primary and preventive medical care to long-term care residents in skilled nursing facilitiesManage residents experiencing acute changes in conditionPerform advanced care planning and chronic care managementPrescribe medications, including controlled substances, in accordance with Pennsylvania lawOrder, review, and follow up on diagnostic tests and treatment plansCommunicate regularly with nursing home staff regarding patient status and care plansCollaborate closely with primary care physicians and interdisciplinary teamsMaintain accurate and timely documentation in the electronic medical record (EMR) Required Qualifications Current, unrestricted Nurse Practitioner license in the state of PennsylvaniaBoard Certified Nurse PractitionerActive DEA licenseClinical background in Adult, Family, or Geriatric specialtiesExperience working in a long-term care or skilled nursing settingProficiency with electronic medical records (EMR)Ability to manage patients with advanced illness and multiple chronic conditions Preferred Qualifications Experience in geriatrics or post-acute careStrong communication and care coordination skillsComfort working independently in a facility-based setting Schedule & Employment Type Monday Friday, 8:00 AM 5:00 PM40 hours per weekFull-Time (5 days/week) Why Join Us?
