Nurse Practitioner (NP) / Physician Assistant (PA) – Long-Term CareA specialized vascular care group dedicated to serving the long-term care setting is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing team. The organization delivers advanced, clinic-level vascular care directly within nursing homes, allowing patients to receive high-quality treatment in a familiar environment.This role involves providing personalized bedside care, reducing the need for patient transport while ensuring comprehensive vascular services—from routine assessments to advanced procedures.Position Overview:The NP/PA will perform on-site clinical assessments and develop appropriate treatment plans for residents in long-term care facilities. This position reports to a Lead Nurse Practitioner and operates within a mobile practice model.Schedule:Full-time (40 hours per week)Key Responsibilities:Conduct on-site clinical assessments and complete comprehensive evaluation documentationPerform detailed histories and physicals for acute, chronic, and complex patientsOrder and interpret diagnostic and therapeutic tests based on patient needsDeliver care in accordance with facility, state, and federal regulationsMonitor and evaluate treatment effectivenessBuild and maintain professional relationships within assigned facilitiesEducate patients, families, and facility staff on services and care plansSupport development of provider and facility partnerships as assignedComplete all required reports accurately and in a timely mannerQualifications:Bachelor’s degree requiredGraduate of an accredited Nurse Practitioner or Physician Assistant programCurrent NP or PA licenseBLS (Basic Life Support) – requiredACLS (Advanced Cardiac Life Support) – preferredCurrent Infection Control CertificationBoard Certified or Board EligibleRequired health documentation: PPD/TB, Hep B, MMR, VaricellaCOVID-19 and influenza vaccination preferredMinimum 1 year of NP/PA experience preferredReliable personal transportation requiredPreferred Experience:Experience in nursing home or long-term care settingsBackground in outpatient vascular careAbility to work independently in a fast-paced environmentCompensation & Benefits:Competitive compensation based on experienceMedical, dental, and vision insurancePaid time off (PTO)Mileage reimbursement401(k) plan
