Job Description:
Summary / Purpose of Role The Physical Therapist at PCRC provides skilled physical therapy services in a long-term care / rehabilitation setting, helping residents improve or maintain function, mobility, safety, and quality of life. You will work with individuals who have various physical impairments and conditions, often geriatric or post-acute, to facilitate safe and meaningful activities of daily living. Key Duties & Responsibilities Evaluate patients' physical function, mobility, strength, range of motion, balance, and gait. Develop individualized treatment plans (care plans), with goals for improving or maintaining the patient's physical abilities. Implement, monitor, and adjust treatment interventions using appropriate physical therapy techniques and modalities. Assist patients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), teaching compensatory or adaptive strategies if needed. Educate residents / patients (and sometimes family or caregivers) on safe movement, transfer techniques, exercises, and preventing falls or injuries. Document evaluations, progress notes, and all therapy-related documentation as per facility, state, and federal regulations. Follow up on patient progress, revising care plans or treatment methods as appropriate. Collaborate with interdisciplinary team (nurses, occupational therapists, speech therapists, physicians, etc.) to coordinate care. Ensure compliance with safety, infection control, and regulatory standards. Provide leadership or mentoring for therapy aides or assistants, if applicable. Participate in facility meetings or continuing education to maintain licensure and keep up with best practices. Qualifications / Requirements Valid Physical Therapy License in the State of Colorado, active and in good standing. Graduation from an accredited Physical Therapy program. CPR / Basic Life Support certification. Experience in geriatric or long-term care / rehabilitation settings is preferred. Strong communication skills, both verbal and written. Ability to work independently and as part of a team. Physical stamina to move, lift, assist patients, sometimes stand for long periods.
