We are looking for a Health Home Care Manager for our Care Management team. This is a field-based position that requires frequent travel to patient's homes in Brooklyn and Queens New York: Department & Position Overview: The Health Home Care Manager will work to outreach and engage potentially eligible patients and obtain appropriate consents to enroll the patient in the Health Home Care Management Program. Once the patient is enrolled, the Health Home Care Manager will work collaboratively with the patient's care team to complete a comprehensive assessment and patient centered care plan, and to coordinate care based on the care plan. Ideal Candidate Will Possess:Associate's degree is a must2 years of Care Management experienceMust be able to travel to minimum of 2 boroughs - Brooklyn and QueensAbility to travel in the field to accompany patients to appointments and meet patients in person when neededStrong working knowledge of local community resourcesDemonstrated ability to work with data reporting, documentation, and outcomesStrong communication and assessment skills; the ability to relate to patients, their families, and community care providers, along with ability to handle rapidly changing crisis situationsAbility to manage high volume caseloadsAble to express empathy and compassion for the underservedExperience navigating several data management systems, such as SalesforceNice-to-Haves:Bachelor's Degree is a plusBilingual Spanish strongly preferredAbility to travel to all 5 boroughs in the NYC areaAccess to a vehicle is strongly preferredWhy work at FreedomCare?We are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $24.00 and $26.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.Compensation Range$24-$26 USD
