This role addresses the emotional, social, financial, and practical challenges associated with cancer diagnosis and treatment, ensuring patients receive compassionate, coordinated, and patient-centered care across the continuum.The Social Worker works collaboratively with oncologists, radiation oncologists, advanced practice providers, nursing, navigation, financial counseling, palliative care, and community partners to optimize outcomes and enhance the overall care experience.Responsibilities:Clinical & Psychosocial SupportConduct comprehensive psychosocial assessments for oncology patientsDevelop individualized care plans addressing emotional distress, coping strategies, and social determinants of healthProvide short-term counseling and crisis interventionSupport patients and families with adjustment to diagnosis, treatment side effects, recurrence, and end-of-life issuesFacilitate advance care planning discussions in collaboration with clinical providersCare Coordination & Resource ManagementIdentify barriers to care including transportation, financial hardship, housing insecurity, and caregiver limitationsCoordinate referrals to community resources, support groups, home care, hospice, behavioral health, and specialty servicesCollaborate with financial counseling to assist patients navigating insurance and coverage concernsAssist with disability applications, FMLA documentation, and related employment issuesMultidisciplinary CollaborationParticipate in tumor boards and interdisciplinary case conferencesPartner with nurse navigators to ensure seamless transitions across the care continuumSupport survivorship planning and transitions from active treatmentCollaborate with palliative care teams when appropriateProgram Development & QualityContribute to oncology quality initiatives and patient experience improvement effortsSupport distress screening programs and ensure timely follow-upMaintain accurate documentation in the electronic health recordTrack key psychosocial metrics to support regulatory and accreditation standardsCore CompetenciesPatient-Centered and compassionate approach to careStrong interpersonal and communication skillsAbility to manage complex emotional and social dynamicsCultural sensitivity and trauma-informed practiceOrganizational skills and ability to manage a diverse caseloadCollaborative team member with a systems-based perspectiveWork EnvironmentOutpatient oncology setting (Medical Oncology and Radiation Oncology)Direct patient contact dailyParticipation in interdisciplinary oncology meetingsRequirements:Master's Degree in Social Work from an accredited programCurrent MYS LMSW or LCSW license requiredMinimum 2-3 years of clinical experience: oncology experience preferredOncology Social Work Certification (OSW-C) required within 12 months of hire dateExperience in distress screening programs and survivorship care preferredStrong knowledge of oncology treatment modalities (medical and radiation)Experience in crisis intervention and short-term therapeutic counselingDemonstrated ability to work in a fast-paced outpatient clinical environment
