Position: LTSS Coordinator / Case Management CoordinatorLocation: New York, NYJob Type: ContractContract: 05/25/2026 - 07/18/2026Schedule: Monday-FridayGuaranteed Hours per Week: 35 HoursPay: $40/hr on W2Job SummaryWe are seeking a detail-oriented and compassionate LTSS Coordinator / Case Management Coordinator to support care management operations. This role involves coordinating non-clinical patient services, conducting member outreach, and ensuring patients receive appropriate care and resources. The ideal candidate will have experience in healthcare coordination, managed care, or case management support.ResponsibilitiesConduct monthly outreach calls to assigned members and complete LTSS & SDOH assessmentsCoordinate non-clinical services including appointments, transportation, medication support, and DMEAssist Case Managers with administrative and coordination tasksEnsure members are connected to appropriate healthcare and community resourcesMaintain accurate documentation in DCMS and other systems in compliance with HIPAAIdentify issues and escalate clinical concerns to Care ManagersSupport patient discharge coordination including transportation and facility placementsCollaborate with interdisciplinary teams to improve patient outcomes and reduce gaps in careAssist with reporting, data tracking, and quality improvement initiativesRequirementsMinimum 4 years of healthcare-related experience (preferred in managed care, case management, or care coordination)Associate's degree required; Bachelor's preferred in healthcare-related fieldStrong communication (verbal & written) and interpersonal skillsExperience with patient/member interaction and telephonic outreachProficiency in EMR systems, DCMS, and Microsoft ExcelAbility to multitask, problem-solve, and work independentlyPreferred QualificationsExperience in managed care or health plans (Medicaid/Medicare)Background in care coordination, community health, or health educationFamiliarity with telephonic case management and patient engagementWhy Work with Pride HealthPride Health offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k) retirement savings, life & disability insurance, an employee assistance program, legal support, auto and home insurance, pet insurance, and employee discounts with preferred vendors.
