Geriatrics Nurse Practitioner

Primary Care Advanced Practice Provider Location: Morris County, NJSchedule: Hybrid, Full-time, Monday FridayAbout the Role:Join a next-generation chronic care team providing value-based, in-home care for patients living with chronic conditions, including kidney disease and related metabolic disorders.
