Job Description:
We are seeking a Nurse Practitioner to provide comprehensive primary care to individuals and families, with a strong focus on health promotion, disease prevention, and coordinated care. This role works collaboratively with clinical staff and other healthcare professionals in a team-based environment. Key Responsibilities Develop and implement effective patient care plansManage patient care to ensure continuity, including follow-ups and coordination of servicesProvide whole-person care, including referrals to specialists and other healthcare providersMaintain accurate, concise, and timely documentation of patient carePerform and document routine health assessments and preventive screeningsEducate patients on diagnoses, treatment plans, medications, and preventive careAdhere to professional standards, including punctuality, timely communication, infection control practices, and patient confidentialityMeet established patient volume expectations for outpatient visitsParticipate in quality assurance and performance improvement initiativesAttend team meetings, clinical meetings, and required trainingsContribute to the development and maintenance of clinical protocols and guidelinesEngage in community outreach and education when requestedRepresent the provider team in meetings or community settings as neededSupport teaching and mentoring of students as assignedMaintain required continuing education for certification and licensurePerform additional duties as assigned Education & Training Graduate of an accredited Nurse Practitioner programMaster s degree in Nursing (MSN or higher)Active Registered Nurse (RN) licenseCertified as a Nurse Practitioner by a recognized certifying bodyPrescriptive authority and DEA certificationCurrent CPR certification Experience 1 3 years of experience as a Nurse Practitioner preferredor significant RN experience in a high-acuity setting (e.g., ER or ICU) Skills & Competencies Strong patient-centered care and customer service skillsExcellent organizational and time management abilitiesAbility to work effectively in a collaborative, team-based environmentStrong written and verbal communication skillsFlexibility and adaptability in a clinical settingBilingual (Spanish) preferred but not required Technical Skills Experience with electronic health records (EHR) systemsProficiency with Microsoft Office and standard office equipment Additional Requirements Valid driver s license and reliable transportationParticipation in required training and quality improvement initiatives
