Remote Nurse Practitioner

As a Nurse Practitioner at CareBridge, you'll have the chance to make a meaningful impact in the lives of individuals in need while working in a flexible and supportive virtual environment. Your Role: Provide essential primary and urgent health care through telephonic and tele-video methods to patients utilizing home and community-based services via state Medicaid programs and dual-eligible members. Work collaboratively with a multi-disciplinary team to develop and implement comprehensive care plans for adult patients facing chronic and complex health conditions. Conduct detailed assessments and create customized treatment plans aligned with each patient's health goals and conditions. Identify and address gaps in patient care to enhance outcomes. Support patients holistically with clear communication and resources, ensuring family involvement in care discussions. Educate patients and families on medication management, illness progression, nutrition, and health maintenance to foster empowerment. Coordinate effectively with clinical team members and other healthcare providers to ensure timely and quality patient care. Maintain precise medical records and documentation in adherence to regulatory and company standards. Engage in continual professional development as required. Prescribe medications in accordance with state regulations. Qualifications: Master's degree in Nursing. Active national Nurse Practitioner certification. Valid, unrestricted Family or Adult Nurse Practitioner license in Ohio. Experience using Electronic Medical Records (EMR). At least 2 years of experience managing complex care cases. Preferred Qualifications: Active Medicaid number in Ohio is highly desirable. Possession of DEA registration or eligibility is preferred. Experience with complex care management for patients with developmental disabilities and chronic illnesses is strongly preferred. Compensation: The salary range for this position is $110,080 to $165,120.
