Nurse Practitioner Indiana PA ( 1 hr E of Pittsburgh)Urgently HiringWe are looking for a Nurse Practitioner to join our medical team in the region of Indiana PA. Our providers work primarily in office based primary care as well as LTC/Nursing Facilities settings providing consultation, primary and follow-up long term care (if desired). We collaborate closely with our patients, their families, rehab staff, physicians, and hospital systems. Our practice philosophy is to have a strong presence in our facilities, to serve the needs of our patients, and collaborate with all those involved in the continuum of care. The incoming NP will be covering mainly office based primary care but will potentially cover locations in the area from Indiana to Punxsutawney PA if so desire, ~30 miles apart. This is a 4-5 day/week position.Job Responsibilities:Office based primary carePerform new admits, consultation, primary and follow-up care for long term care residentsRound on patients, track patient progress, remove obstacles to functional progress, and improve quality of life.Engage with clinical staff on a regular basis and maintain a professional, collegial relationship with other facility physicians, caregivers, administrators, and directors.Engage with patients and families during care and communicate clearly to promote understanding of treatment options and procedures. Set expectations with facility, patients, and families. Attend facility team meetings as needed Documentation: Follows protocol and guidelines for documentation, billing, and coding.Regularly meet with facility leadership to understand their needs and build relationships.  Report back to Director facility happenings and updatesActively collaborates with treating physicians, providers and treating groups(referrals, wound care, Orthopedists, etc.)Requirements:Graduation from an accredited Nurse Practitioner ProgramActive/Current NP license in PAPotential to travel between multiple long term care facilities (if so desired)Schedule:Full time (4- or 5-day per week)Compensation (range):Up to $125k to start plus production bonusesBenefits:Production Bonus (value and performance-based comp plans)Health Insurance401k with 3% matchFlexible scheduleWe are looking for a Nurse Practitioner to provide direct patient care within our primary care office as well as Nursing/Long term care facilities, can effectively communicate with patients, who enjoys working collaboratively with other healthcare professionals and administrative staff.
