About the Role As a Matrix provider, you will conduct invaluable health assessments in a one-hour visit that reviews a member's current health status, medical history, medication adherence, social environment, and associated risks. Your role will include: Performing adult and geriatric assessments, diagnosing and treating health conditions, providing health education, making physician and case management referrals, and maintaining thorough documentation as per Matrix guidelines. Administering and ordering necessary lab tests and point-of-care diagnostics. Collaborating with physicians, case managers, social workers, family members, caregivers, and other medical personnel. Partnering with Primary Care Physicians (PCPs) to enhance patient education and facilitate follow-up care. Delivering care in patients' homes or Skilled Nursing Facilities. Benefits of Working at Matrix Consistent Employment Schedule - Work full-time typically Monday to Friday from 8:00 AM to 5:30 PM, with assessments conducted from Sunday to Saturday, 8 AM - 7 PM. Competitive Salary - Earn a base salary between $105,500 - $168,400 based on experience and market analysis. Incentive Program - Participate in a monthly (uncapped) Clinician Traveler Incentive Plan, with average payouts of $500 per month for those who qualify. Licensure Support - We assist you in obtaining necessary additional state licensure and credentialing as needed. Qualifications Master's Degree or equivalent experience with valid NP licensure. Current NP licensure in your practice state, including prescription authority or ability to obtain it. Certification from AANP, AACN, or ANCC in relevant specialties. Current BLS, ACLS, or CPR certification. At least one year of experience as a Nurse Practitioner. Able to travel up to 80-90% of the time, including overnight trips and weekends within the assigned states. Valid state driver's license and automobile insurance coverage as mandated. Proficient in using health/medical software and strong communication skills.
