This role focuses on supporting individuals experiencing homelessness who are transitioning from hospital, emergency department, or clinic care and require short-term medical oversight and stabilization.Responsibilities include conducting intake assessments, managing and continuing treatment plans, monitoring acute and chronic conditions, and addressing new medical concerns as they arise. The provider will collaborate with primary and specialty care teams, participate in care conferences, support discharge planning, and utilize motivational interviewing and de-escalation techniques.
