As a Registered Nurse (RN), you will supervise, evaluate, and deliver nursing services to clients in various senior living communities, including assisted living and independent living facilities. Our facility-based RNs benefit from: Providing personalized care within the beautiful environments of assisted living facilities 1:1 patient care Continuity of care from patient admission to discharge Enhabit RNs work collaboratively with facility wellness staff daily to ensure effective communication and continuity of care for residents and their families. You will also consult with physicians and office staff regarding patient care, acting as the primary point of contact for all care disciplines involved with patient support. Key responsibilities include performing accurate OASIS collection, maintaining current medication profiles, timely reporting of lab values to physicians, and attending weekly case conferences and monthly case manager meetings.
