We are passionate about helping individuals experiencing depression; anxiety; schizophrenia; behavioral changes related to Alzheimer's disease or other dementias, medication management or substance abuse; and other mental health issues. /n/n/n As a member of the multidisciplinary treatment team, our therapists provide direct clinical services - including assessments, interventions, treatment planning and discharge planning, as well as providing individual, family, and/or group therapy to further patient/family/group progress in treatment. /n/n /n/n Members of the Oceans' team work together to provide the highest quality care by practicing the "Oceans 6" values. /n/n /n/t * Dignity /n/t * Comprehensive Care /n/t * Quality /n/t * Integrity /n/t * Advocacy /n/t * Teamwork /n /n/n /n/n Essential Functions: /n/n /n/t * Provides direct clinical services in accordance with accepted standards of social/work clinical practice. /n/t * Plans and conducts individual family and/or group therapy as assigned by the Director of Social Services or physician's order. /n/t * Conducts, assesses and interprets completion of Comprehensive Integrated Assessment. /n/t * Reviews all Multidisciplinary assessments and compiles a Multidisciplinary Treatment Plan under the supervision of the attending physician. /n/t * Performs case management and discharge planning as assigned by the Director of Social Services. /n/t * Attends multidisciplinary treatment team meetings, provides updates on assigned patients and their progress toward treatment goals; combines the reports of all disciplines into a multidisciplinary treatment integration summary, which facilitates the patient's progress towards treatment plan goals /n/t * Delivers patient services in an ethical manner which promotes respect for the patient and enhances the patient's dignity and satisfaction; Demonstrates the ability to assess and interpret a patient's response to treatment in age/disability/culturally sensitive specific manner. /n/t * Assesses and interprets age/disability/cultural needs; Identifies safety, psychosocial, special mobility and special equipment needs; and Communicates in an age/disability/culturally sensitive related manner to patients and their families /n/t * Conducts treatment and discharge planning in accordance with age/disability/cultural needs and involves members of the patient's support system in decision making as appropriate. /n/t * Performs other duties and projects as assigned. /n /n",
