The Resource Center has been providing services to persons with disabilities in the Chautauqua County area since 1958. From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities. The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll. Personal Outcome Measures Interviewer - JOB DESCRIPTIONRESPONSIBLE TO: QA/QI Supervisor FUNCTION: To assist the organization in helping people discover and define their own quality of life as well as identify quality of supports provided to people supported by the organization by conducting POM (Personal Outcome Measures) interviews REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Strong observational skills. Must have the ability to multi-task with strong organizational skills. Demonstrates strong interpersonal skills with patience and empathy. Independent thinker with the courage to stand alone and report unbiased information. MINIMUM QUALIFICATIONS: High school diploma or equivalent and 5 years' experience working with people with disabilities and/or mental health diagnoses. Associates degree or DSP/FLS Certification preferred. Ability to become CQL Certified as a POM interviewer within 1 year on the job and ability to maintain the certification.IDENTIFIED KEY COMPETENCIES: Competencies are developed and detailed in this section by Card Sort. DUTIES Coordinates all interview activities including scheduling, obtaining consents, conducting interviews and outcome determinations. Documents outcome determinations in CQL portal. Documents and provides recommendations to person's team based on interests discovered during the interview process. Provides valid/reliable data using instructions provided by CQL. Provides data monthly to the CQL Steering Committee. Facilitator of "Dignity and Respect" Basic Assurance factor group and coordinates action plan required of accreditation.
