JOB OVERVIEW • Job Title: Nurse Practitioner Primary Care / Physician Assistant Primary Care • Job Type: Locum Tenens • Location: Hugo, CO • Service Setting: Outpatient Clinic (Primary Care / Family Medicine) • Coverage Type: Clinical Only • Coverage Period: 10/6/2025 3/31/2026 (3 6 months) • Clinical Shift Schedule: Tues, Thurs, Fri 8a to 4p ( hours per week, includes admin time)COVERAGE DATES • Flexible scheduling within the coverage period (October 2025 March 2026)PATIENT INFORMATION • Patient Demographics: Adults and Family Practice • Patient Volume/Census: Approximately 20 patients per day • Case Mix: Primary care, family medicine, joint injections, toenail removals, lesion removalsFACILITY INFORMATION • Facility: Lincoln Health Byers Clinic • Address: 111 6th Street, Hugo, CO 80821 • Specialty Backup Available: Other Lincoln Health providers rotate coverage at nearby clinics • Support Staff: Local clinical team support • Reason for Coverage: Temporary coverage while onboarding a new provider • Additional Details: Clinic is currently free-standing but in process of becoming a Rural Health ClinicCOMPENSATION & BENEFITS • Contract Type: Daily OT after 8 hours; Weekly OT after 40 hours (based on fixed) • Travel, Lodging, and Malpractice Insurance: Covered (Malpractice COI required) • Mileage: Reimbursed per IRS rates • Lodging: Cap at $150/night (non-preferred hotels), preferred hotel list available • Rental Car: $50/day cap (Apr Nov, mid-size), $75/day cap (Dec Mar, mid-size 4WD SUV) • Airfare: Economy seating, 1 checked bag; no upgrades allowedREQUIRED PROCEDURES • Joint injections • Toenail removals • Lesion removalsJOB REQUIREMENTS • Licenses: Active Medical License in Colorado • Board: Board Certification if applicable • DEA: Current DEA registered in Colorado • Certifications: ACLS, ATLS, PALS, BLS (must be current and not expiring within 30 days) • Experience: Prior primary care or family medicine experience strongly preferred • Other Qualifications: Capable of seeing 20 patients/day; ideally 5+ years experience • Additional Documentation: Malpractice insurance (COI), CME credits (last 24 months), case logs, CAQH, CV, diplomas, driver s license, headshotDUTIES & RESPONSIBILITIES • Provide primary care and family medicine services in outpatient clinic setting • Perform procedures such as joint injections, toenail removals, and lesion removals • Deliver comprehensive care to approximately 20 patients per day • Work independently in a free-standing clinic setting with occasional coverage support • Collaborate with Lincoln Health system clinics in Byers, Limon, Hugo, and Flagler as needed
