With a mission focused on improving access and health outcomes, the team works collaboratively to deliver coordinated, patient-centered care without financial barriers.What Your Week Will Look Like: Provide comprehensive primary care through preventive visits, chronic disease management, and same-day needs, with 30-minute appointment slots.Build ongoing relationships with patients and play a central role in their long-term care.Collaborate with an integrated team—including behavioral health, pharmacy, and dental—to support whole-person care.Develop and adjust treatment plans, partnering with patients in their care decisions.Manage a balanced patient panel across acute and chronic conditions, including behavioral health concerns.Coordinate referrals and follow-up care with support from a collaborative care team.Review results, monitor progress, and document care in the EHR.Participate in quality improvement and contribute to a team-based care environment. Primary Care Physician / PCP Qualifications:Qualified & Licensed: MD or DO from an accredited medical school with completion of a Family Medicine or Internal Medicine residency. Board certification or active pursuit of certification in a primary care specialty is required.
