Remote Health Risk Assessment Provider

We are looking for compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) for an exciting PRN opportunity focused on performing in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Your Key Responsibilities: Conduct comprehensive in-home HRAs for Medicare Advantage patients. Review patients' medical histories, current medications, lifestyle choices, and significant social factors. Perform basic physical and mental health assessments. Provide preventive care education without the need for ongoing patient management. Travel locally to conduct assessments within your county and nearby areas. Requirements: Must possess an active NP or PA license (note: Pediatrics, PMHNP, and Women's Health specialties are excluded). Availability to work a minimum of eight (8) full 7.5-hour days monthly. Willingness to travel within approximately 60 minutes of your home. Additional Information: As a 1099 contractor, you may write off mileage at the rate of $0.725 per mile in 2026 (please consult a tax advisor for more details).
