LTC Physician Care, PC is a medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships. LTC is seeking dedicated Nurse Practitioners to join our team of Primary-Care Providers in Forest Hills and Lamont, MichiganWe are happy to consider new graduates!Requirements and Qualifications Family Medicine, Primary Care, or Geriatrics is strongly preferred.Candidates must hold a valid and unrestricted APP medical license in Michigan.
