Position Type: Full-Time (37.5 hours per week, 52 weeks per year) - Grade 13 Pay Range: $17.26 - $22.45Monday to Friday: 8:30am - 4:30pmJOB SUMMARY:As a Self-Direction Support Broker, you'll directly empower people to live more independently, build meaningful relationships, and achieve their personal goals. You will assist individuals hey support to gain access to services and programs and to coordinate the use of those services and programs. The Self-Direction Broker provides person centered services, on-going support, advocacy, and regular follow-up to ensure the person's maximum benefit from services and programs.What You'll Do:You will partner with individuals, families, and circles of support to help people build the life they want. In this role, you will:Facilitate person-centered planning sessions and help individuals build their Circle of Support.Assist participants in developing, modifying, and implementing their Self-Direction plan and budget.Support individuals in understanding their responsibilities within Self-Direction and encourage independence.Help identify meaningful community connections aligned with personal goals.Assist individuals with recruiting, monitoring, and hiring their own staff, when requested.Monitor monthly expenditures to ensure budgets are on track.Complete and submit Self-Direction budgets for FI and DDRO approval.Lead launch meetings and facilitate at least two Circle of Support meetings annually per participant.Complete Community Habilitation Plans for those utilizing self-hired Com Hab services.Maintain accurate, complete, and confidential case files.Ensure compliance with OPWDD regulations, agency policies, and incident management requirements.Use and support individuals in navigating eVero software for documentation.Build strong partnerships with service providers, FIs, DDRO liaisons, and families.JOB REQUIREMENTS:Professional, dependable, and able to communicate clearly in writing and in person.Warm, non-judgmental, and supportive when working with individuals and families.Self-motivated and comfortable working independently.Skilled in facilitating informed choice, self-advocacy, and problem-solving.Able to remain calm and effective during crises or conflict.Proficient with computers, including Excel, Word, PowerPoint, and eVero or similar systems.Comfortable managing and understanding budgets.Willing to maintain all required certifications, including Broker Institute Training and annual 12-hour recertification.Posseses a valid NYS Driver's License (within agency standards) and a reliable, insured vehicle; must be able to maintain certification as an Agency driver.MINIMUM QUALIFICATIONS:You must meet one of the following:Associate's Degree in a related field, and/or1-3 years of related experience, including some leadership responsibility, orAn equivalent combination of education, training, and experience.
