Job Description:
The VA Central California Health Care System is currently recruiting for one full time Clinical Pharmacy Specialist (Rounding-Inpatient) position with Pharmacy Service located at the Fresno, Facility. The Clinical Pharmacy Specialist provides clinical coverage for Acute Care settings and special procedures such as OR, PACU, GI lab, Ambulatory Infusion/Oncology, Nuclear Medicine, ED as well as provides coverage as needed for inpatient operations, specialty care, mental health and CLC. The incumbent will carry a scope of practice and work directly with the acute care medical teams performing medication reconciliations, monitoring labs and pharmacokinetics, making therapeutic adjustments, providing discharge counseling, while also serving as a primary preceptor for our robust pharmacy education program, working with our interns and residents. Major duties include but not limited to: * Provide direct patient care and will be qualified and authorized to provide services in inpatient clinical wards/units, Emergency department, long-term care and virtual care settings as outlined in the CPS scope of practice. * Provide appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. * Recognize physical signs and clinical symptoms relevant to the patient's drug therapy. * Follow-up with providers and or other interdisciplinary staff to discus the plan of therapy if necessary * Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing. Monitoring will include prospective review and intervention in: * Therapeutic appropriateness of a patient's drug regimen * Therapeutic duplication in the patient's drug regimen * Appropriateness of the route and method of administration * Degree of patient compliance with the. prescribing regimen * Drug-drug, drug-food, drug-laboratory, or drug disease interactions * Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects * Oversee pharmacotherapy to ensure best possible evidence based care is provided following national treatment guidelines in addition to adherence to national formulary and local policies. * Document adverse drug events, medication errors, allergies, clinical interventions, drug information provided, patient therapy evaluations, and counseling in the electronic medical record (e.g., CPRS, VISTA, JPSR, and/or VADERs) * May be required to assist in the operation in other areas of Pharmacy Service, including distribution and clinical work in other areas (i.e., medication order review and verification) as assigned * Provide clinical support and expertise for pharmacy admissions/discharge medication reconciliations, including but not limited to care coordination with SNFs and other community providers * Works as a team with providers, including physicians, PAs, ARNPs in the design and continued evaluation of the Veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy. * Serves as an authoritative information source on medications and their utilization in therapy. * Evaluates drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs. * Performs other duties as assigned.
