Primary Care Nurse Practitioner (NP) Location: South Frankfort, KYSchedule: Monday Friday 8:00 AM 5:00 PM EST also open to 4 days per week Position Overview We are seeking a skilled and compassionate Primary Care Nurse Practitioner (NP) to provide comprehensive medical care to patients residing in a skilled nursing facility. This role focuses on managing acute and chronic conditions, promoting preventive care, and improving overall patient outcomes within a collaborative, interdisciplinary environment. Key Responsibilities Provide primary care services for long-term care patients in a skilled nursing facilityPerform comprehensive patient assessments, diagnoses, and treatment planningPrescribe medications, including controlled substances, in accordance with Kentucky state lawOrder, review, and follow up on diagnostic tests, labs, and imagingManage chronic conditions such as diabetes, hypertension, COPD, and heart diseaseAddress acute medical concerns and changes in patient conditionConduct routine wellness visits and preventive care servicesCommunicate regularly with nursing home staff regarding patient status and care plansCollaborate with primary care physicians and interdisciplinary care teamsMaintain accurate and timely documentation within the EMR system Required Qualifications Current, unrestricted Nurse Practitioner (NP) license in the state of KentuckyBoard Certification (FNP, AGNP, or related specialty)Active DEA licenseClinical background in adult, family, or geriatric careExperience working in a long-term care or skilled nursing settingProficiency with electronic medical record (EMR) systemsAbility to manage patients with advanced illness and multiple chronic conditionsStrong collaboration skills within an interdisciplinary care team Preferred Skills Comprehensive patient assessment and diagnostic expertiseChronic disease managementAcute care evaluation and treatmentMedication management and prescribingOrdering and interpreting labs, imaging, and diagnosticsPreventive care and wellness visitsCare planning and follow-upPopulation health management Compensation & Benefits Competitive salary + bonus opportunitiesHealth, dental, and vision insurancePaid time off (PTO)Malpractice insurance coverageContinuing education creditsRecertification reimbursementDisability insurance401(k) with employer match Why Join This Opportunity?
