Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for. Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD Monday-Friday 8-5 A mix of peds/adults A decision is typically made within 7-14 days APCs with 3 years of experience in a similar setting PAs must have a CA Controlled Substance Education Course Certificate NPs must have a Furnishing License with Schedule II An active DEA is needed before the start Credentialing takes 30-90 days The following immunizations are required for credentialing (before the start date): Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30-day notice for ending all assignments.
