Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.Active Indiana Physical Therapy license is required to start the assignment.BLS certification from AHA or ARC may be required.No new graduates. Minimum prior clinical experience in outpatient physical therapy required.Experience evaluating patients, developing and implementing treatment plans, and documenting per clinic and payer requirements.Experience with functional treatment programs and safe patient handling techniques.Ability to supervise, delegate, and provide direction for discipline-specific therapy services.Maintain professional licensure in good standing and complete state-mandated continuing education as required.Preferred: experience with outpatient documentation systems, goal writing, letters of medical necessity, and charge entry. Assignment Details Facility Type: Outpatient.Duration: 13 week assignment.Shift Pattern: 5x8 schedule.Positions Available: 2.Candidates must adhere to clinic standards for personal appearance, attendance, punctuality, and safety practices.
