Preference is they see infants but 12+ is requiredA decision is typically made within 7-14 daysAPCs with 3 years of experience in a similar setting PAs must have a CA Controlled Substance Education Course Certificate NPs must have a Furnishing License with Schedules IIActive DEA needed before the startCredentialing takes 30-90 daysThe following immunizations are required for credentialing (before the start date): Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
