This role acts as a resource for CHWs regarding FIM models, chronic disease prevention, and health-related social needs, while addressing daily implementation challenges. Through individual and group coaching, strengthen CHW skills, confidence, and role clarity across diverse clinical and community settings. Develops and delivers initial and ongoing training for CHWs focused on FIM workflows, documentation requirements, professional boundaries, and best practices. This includes supporting onboarding of new CHWs and adapting training content to meet community needs. Working with clinical partners, community-based organizations, and program leadership to support effective integration of CHWs into care teams. Including clarifying CHW roles, communication pathways, referral processes, and bridging community and clinical systems to improve coordination and effectiveness. Develops education and supports the engagement of Community Health Workers (CHWs), healthcare providers, the general public, and other relevant stakeholders in Kansas. Collaborates with partners at the local, state, and national levels to align community health worker initiatives with Food is Medicine and chronic disease programs, promoting them both locally and statewide. Creates and maintains effective educational activities and materials. Works with the community health worker workforce to gather insights, ideas, and needs to help expand their knowledge, resources, and sustainability in Kansas. Job Responsibilities may include but are not limited to the following: Provides technical assistance to state and local agencies, organizations, and coalitions to support the implementation of program and grant strategies. Supports partners in developing and executing successful activities to ensure program success and the completion of deliverables as outlined by funder(s). Develops and oversees strategies for improving performance on standards and maintaining adherence to quality work, including clinic/community-based organization site visits to identify and address technical assistance needs. Offers technical assistance to state and local agencies, organizations, and coalitions for the development, training, and implementation of CHWs and FIM. Plans, organizes, and facilitates activities related to and manages the resources necessary to maintain and expand the Community Health Worker Program. Writes and negotiates contracts, manages contractors, and creates strategic program and evaluation plans that result in completed grant activities that align with funder objectives. Maintains complete and accurate program records. Provides guidance for evaluation planning and works with the epidemiologist(s) on maintaining necessary data and evaluation plans. Qualifications * Education: * Licensing & Certification: * Valid Driver's License - Incumbent is required to have and maintain a valid driver's license when operating a state vehicle, a private vehicle, or a rental vehicle for the benefit of the State. * Minimum Qualifications: * One year of experience as a Community Health Worker or Community Health Worker Ally. The ability to provide technical assistance across diverse settings, use of basic data systems, and document tools. * Preferred Qualifications: * Bachelors Degree related to Public Health, Health Promotion, Health Education, Health Administration, Social Work, Allied Health * Word, Excel, PowerPoint, Access * Proofreading, editing, attention to detail * Experience in technical assistance, program evaluation, training, and/or grants manage * Ability to research, organize and present health information and promotional materials * Ability to develop and deliver effective presentations for training and educational purposes * Experience working in or with health care field and/or experience working with Community Health Work * Experience coordinating or providing support to community programs * Excellent interpersonal skills, ability to build and maintain professional relationships * Post-Offer, Pre-employment Requirements: * Kansas Tax Clearance Certificate: A valid Kansas Tax Clearance Certificate is a condition of employment for all employees of the State of Kansas.
