What You’ll Do:Administrative & Admissions SupportPrepare, submit, and track Admission Notices, Death Notices, and other required documentationAssist with referral intake, including gathering information from hospitals, facilities, and community partnersCoordinate and schedule initial nursing assessments and admissionsMaintain accurate and up-to-date admission packets, forms, and compliance documentsSupport completion of admission paperwork with patients, families, and facility staffMaintain organized, accurate records and documentationCommunity & Facility CoordinationTravel occasionally between Las Vegas, Española, and Santa Fe to support facility partners and internal teamsSet up and coordinate education events, in-services, and informational sessions at partner facilitiesAssist with outreach efforts to strengthen relationships with referral sourcesProvide clear explanations of hospice services to patients, families, and community contactsHelp identify alternative services when a patient is not eligible for hospiceQualifications:At least one (1) year experience in homecare, home health, hospice, and/or healthcare-related field preferredPrior community outreach experience is a plusStrong administrative and organizational skills, with the ability to manage multiple time-sensitive tasksComfortable traveling regularly between Las Vegas, Española, and Santa FeProfessional communication skills, including the ability to speak confidently with facility staff, patients, families, and referral partnerSelf-motivated, dependable, and able to work independently while also collaborating with a teamComfortable presenting information or assisting with education events, in-services, or facility trainingsAbility to complete necessary documentation appropriately, legibly, and timelyAbility to handle stressful and delicate situationsInterpersonal, verbal, and written communication skills necessary to complete job responsibilitiesValid driver’s license and proof of insurance are required#ACHOSEmployee wellbeing is top priority at Addus Homecare, and we're thrilled to announce our recognition as the top healthcare company on Indeed's 2024 Top 100 Work Wellbeing Index.
