SUMMARY:Provide quality and cost-effective primary care services to patient population, meeting their diverse health and social needs. The Chautauqua Center (TCC) expects all employees to respond and participate to emergency situation per emergency policies and procedures. DUTIES AND RESPONSIBILITIES: ClinicalPer specialty, perform complete examination of patients including adolescence, children and infantsAssess and manage acute and chronic conditions and participate in disease registryScreen all patients for health maintenance needs, including immunizationsEducates patients and family members in health promotion, disease prevention and birth control methods as appropriateAssists in preparation and selection of patient education materialsInitiates diagnostic studies referring abnormal results of concern to Medical Director of specialistHelps coordinate efficient flow of patients through the system of careHelps monitor status of referralsObtain and review patient histories and develops patient care charts, ensuring completeness and accuracyHelps facilitate creation and tracking of patient data, specimens and recordsPractices safety, environmental, and/or infection control methodsHelps provide triage services and treatment services during disaster, per protocolMaintains hospital privileges as designated by TCCShares a call schedule with other providers Other duties as assigned by TCCManagementProvide a proportionate share of daily supervision and be able to act as mentor to PA/NP and co-sign their chartsProvide consultative support, including differential diagnosis to NP, PA, mid-level Providers, triage nurse and other PhysiciansParticipate in proctoring of new hire and newly graduated ProvidersOther duties as assigned by TCCAdministrativeProvide trainings on procedures, risk management and emergency careParticipate in Quality Assurance, peer review and maintain confidentialityParticipate in quality improvement, chart audits, and process improvement programs both system wide and cluster specificParticipate in cluster specific meetings and administrative task as assigned by Medical Director or cluster lead CliniciansHelps in recording of no-show visits and lab results/refillsParticipates or collaborate with marketing and other health education staff on education and patient recruitment health fairsMaintain current knowledge-base and appropriate licensurePerforms miscellaneous job-related duties as assignedOther duties as assigned by TCC QUALIFICATIONS: Licensed to work in the state of New York as a Medical Doctor Board CertificationCPR certifiedKnowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertiseMinimum 1-year experience in a primary care clinicLegal authorization to work in the United States A valid State Driver's license with clean records and access to insured automobile
