About the Role As a Nurse Practitioner with Matrix, you will conduct thorough, insightful health assessments in the comfort of our members' homes or Skilled Nursing Facilities. Each assessment, lasting up to one hour, allows you to delve deeply into the members' health, medical history, medication adherence, and social circumstances, ultimately elevating the quality of care provided. Your key responsibilities will include: Performing adult and geriatric assessments, including medical history, diagnosis and treatment, health education, and referrals. Conducting and ordering necessary point-of-care and lab-based testing. Collaborating with physicians, case managers, social workers, and family members to ensure comprehensive care. Providing ongoing education and support to members to effectively address their individual needs. Licensure Support - We can assist you with any additional state licensure and credentialing needs. Qualifications A Master's Degree or equivalent experience with NP licensure. Active NP licensure with prescriptive authority or readiness to obtain as required. AANP, AACN, or ANCC board certification in an applicable specialty area. Current BLS, ACLS, or CPR certification. At least one year of NP experience. Willingness to travel 80-90% of the time, including overnight stays and weekend work across your assigned state and potentially neighboring states. A valid driver's license and appropriate automobile insurance coverage. Strong computer skills and familiarity with health/medical record software.
