JOB TITLE: SOCIAL SERVICES COORDINATOR DEPARTMENT: ADMINISTRATION REPORTS TO: ADMINISTRATOR JOB PURPOSE The Social Services Coordinator is responsible for providingSocial Services interventions which successfully address residents' needs andto link social supports, physical care and physical environment with residents'needs and individuality in accordance with all state and federal regulations. KEY RESPONSIBILITIES • Communicate in a positive manner with residentsand their family/responsible party in the resolution of problems, to provideverbal counselling, and to communicate resident's physical, mental andpsychosocial needs to family member/responsible party, staff members,consultants and/or other professionals • Complete and maintain a comprehensive assessmentincluding social history, a plan of care and progress notes for each residentas required by the facility and state and federal regulations • Maintain individual residents' rights forprivacy, treatment schedules, property, civil rights and all other rights asdesignated by state and/or federal law. • Maintain confidentiality about residents' pastand present care and/or other information • Communicate in a positive manner with healthcareteam members, residents, families, visitors and vendors • Adhere to facility policies and procedures,including infection control, safety, and HIPAA regulations • Responsible for maintaining a calm, supportiveand caring environment for residents and staff • Responsible for complying with all safety andsanitation procedures including universal precautions • Responsible for attending in-service programs,educational workshops and meetings as required by the facility and state • Perform other duties as assigned QUALIFICATIONS • Must be a graduate of an accredited college oruniversity with a major in social services. • Must have a current license to practice as asocial worker in the State of Ohio. • At minimum 2-years' experience as a socialworker in a health care setting preferred • Excellent problem-solving skills and ability tomulti-task and prioritize tasks • Excellent customer service and communicationskills • Ability to maintain required documentation • Ability to follow written and verbalinstructions and to work independently and as a part of a team • Ability to remain calm and cope with the mentaland emotional stress of the position • Must be in good general health and free ofcommunicable diseases • Must have visual acuity and ability to hear,with or without use of prosthetics • Must have the physical ability to sit, stand,walk, bend, and move intermittently throughout the shift, as well as theability to climb and descend stairs, use elevator and assist residents • Must have the ability to read, write and speakthe English language WORKING CONDITIONS • Works under the supervision of the facilityAdministrator • Subject to hostile, aggressive, emotionallyupset residents, who may not be in control of their behavior. Subject toemotionally upset family members related to resident's condition, care, declineand/or services • Subject to unpleasant odors and to possibleexposure to infectious waste, disease or conditions • May be required to work beyond normal workinghours, on weekends and holidays and on other shifts when necessary.
