Hospice/Palliative Medicine Nurse Practitioner

UChicago Medicine AdventHealth is hiring for their Community Palliative Care program. This is a growing program which is so needed in our communities. The program serves our homebound patients, giving them access to palliative care in the comfort of their homes. 1-3 years of APP experience, preferred Palliative Care Certification, preferred Weekly Monday - Friday schedule No call requirement Outpatient palliative care in patient s homes Visit patients in assigned territory Work collaboratively with Social Workers and House Calls Physicians & APP's Competitive compensation, benefits, CME and occurrence-based malpractice insurance Annual Salary: $110K - $120K REQUIRED QUALIFICATIONS: Education/Training Graduate of an accredited school of nursing and nurse practitioner program Masters degree in Nursing or Physician Assistant Studies Current Illinois license in good standing Certified as a nurse practitioner or physician assistant by an approved certifying organization.
