Experience with a larger med pass Job Summary We are seeking a dedicated and compassionate Behavioral Health Licensed Vocational Nurse (LVN) to join our multidisciplinary team. The ideal candidate will possess a strong background in behavioral health nursing, with experience in managing patients with complex mental health needs, developmental disabilities, and co-occurring medical conditions. As a Behavioral Health LVN, you will provide direct patient care, support treatment plans, and assist in creating a safe and therapeutic environment for individuals across various care settings. Responsibilities Conduct comprehensive patient assessments, including vital signs, medication administration, and documentation of behavioral and physical health status. Assist with developing and implementing individualized care plans tailored to patients' mental health needs and medical conditions. Support medication management, including injections, IV infusions, and other prescribed treatments while adhering to safety protocols such as HIPAA and aseptic techniques. Monitor patients' responses to treatments, report changes in condition promptly, and collaborate with physicians and interdisciplinary team members for optimal care. Provide emotional support and behavioral interventions for individuals with developmental disabilities, dementia, Alzheimer's disease, or memory care needs. Maintain accurate medical records using electronic health record (EHR) systems such as Epic or Cerner; ensure compliance with documentation standards. Assist in patient education related to medication administration, disease management, and health coaching strategies. Ensure infection control procedures are followed rigorously; manage specimen collection/processing when necessary. Support discharge planning by coordinating post-acute care or community-based services for patients transitioning from inpatient settings or assisted living facilities. Qualifications Valid LVN license with current registration in the state of practice. Prior experience in behavioral health settings preferred; experience working with individuals with developmental disabilities or dementia is highly desirable. Knowledge of EMR/EHR systems such as Epic, Cerner, or Athenahealth; proficiency in medical documentation and ICD coding (ICD-9/10). Strong understanding of patient assessment techniques, vital signs monitoring, medication administration (including injections), and basic life support (BLS). Familiarity with various care environments including long-term care facilities, assisted living communities, hospitals (including ICU and trauma centers), hospice care, and outpatient clinics. Ability to work effectively within multidisciplinary teams while maintaining professionalism and compassion. Excellent communication skills combined with critical thinking abilities to triage effectively and respond swiftly to urgent situations. Knowledge of physiology, anatomy, infection control practices, and medical terminology relevant to behavioral health nursing.
