In this rewarding role, you will provide advanced palliative care to patients within their homes, helping to enhance their quality of life while expertly managing complex medical situations. You will work autonomously, closely collaborating with the Palliative Medical Director to deliver outstanding care and support to patients and their families throughout all stages of treatment. Key Responsibilities: Team up with the Palliative Medical Director and care team to provide comprehensive, patient-focused palliative care. Conduct thorough patient assessments covering physical, emotional, and psychosocial aspects. Collaborate with patients and families to create and implement personalized care plans. Oversee and modify treatment plans according to patient needs and advancements. Prescribe medications, including controlled substances, and order diagnostic tests in accordance with state laws. Ensure timely and accurate clinical documentation that meets professional standards. Maintain clear communication with physicians, caregivers, and other healthcare professionals. Participate in evening and weekend on-call rotations as necessary. Coordinate referrals for additional care services when required. About You: Specialized Knowledge/Skills: Strong leadership as well as interpersonal, written, and verbal communication skills. Proficient independent decision-maker with self-motivation. Knowledge of palliative medicine principles, home health, hospice eligibility, and Medicare/Medicaid regulations. Familiarity with the interdisciplinary care model. Compliance with continuing education requirements for Gentiva and licensing boards. Skilled in using point-of-care technology and software. Education/Experience: Master of Science in Nursing from an accredited nursing school (or equivalent as per state law). A minimum of two years' experience as a Nurse Practitioner. Preferred specialization in Family or Adult Medicine. Experience with Part B billing. Ability to operate independently in an advanced practice role. Licenses/Certifications: Current RN license and Nurse Practitioner certification as required by state regulations. Collaborative agreement with a supervising physician (or willingness to create one). Part B Provider certification. Federal DEA license and state DEA license (if applicable). ACHPN certification in Hospice and Palliative Care preferred. A valid driver’s license and current automobile insurance.
